DESIRED COMMUNITY OUTCOMES 

United St. Croix Valley will build stronger communities by identifying needs and specifying desired outcomes for all funded programs.  The public will be aware of, and access, the programs and services available in the community.  People in need of programs and services will receive assistance in a timely fashion through access to United Way's 2-1-1, twenty-four hour telephone information and referral service.

 

GOAL - Meeting Basic and Emergency Needs

All individuals and families will have the basic necessities of food, shelter and protection during emergencies.

 

Outcomes

Low income individuals and households have their basic needs met.

Low income individuals and families reduce their dependence on emergency services and increase their economic self-sufficiency.

bullet

Individuals needing basic skill education will receive it, thereby becoming better employees with opportunity for advancement.

bullet

Employed individuals needing basic skills education will receive it, thereby becoming better employees with greater opportunity for career advancement.

bullet

People will have basic literacy skills.

bullet

Communities are prepared to address emergencies.

bullet

Resource information is readily accessible through 24-hour telephone information and                       referral service, 2-1-1.
 

 

GOAL - Strengthening Children and Families.

Children and youth will develop the skills, self-esteem and resilience needed to become healthy adults and productive citizens.  Families will have strengthened relationships and the ability to resolve personal problems and crises.

Pregnant women utilize prenatal care.

Children/youth are socially, cognitively and physically ready for school.

bullet

Children/youth and their parents or other caring adults have stable, supportive relationships.

bullet

Children/youth improve their school performance.

bullet

Children/youth can solve their own problems non-violently and make good decisions.

bullet

Children/youth, adults and families cope with or solve personal, emotional and/or behavioral problems.

Parent of at-risk children improve parenting skills and practices.

bullet

Families manage household budgets and finances effectively.

bullet

The incidences of domestic and sexual violence are reduced.

All have shelter and protection from unsafe conditions and communities understand how to help prevent violence.
 

 

GOAL - Promoting Health and Self-Sufficiency.

All individuals and families will have improved physical health through access to wellness education.  Senior citizens and person with disabilities will experience enhanced independent living and self-sufficiency.

 

Outcomes

Individuals, families and healthcare providers have increased knowledge of the nature of chronic diseases and how to prevent and./or cope with them.

Children/youth and adults understand and choose healthy lifestyle practices and avoid, drugs and alcohol abuse.

Individuals are able to remain in their own homes; avoid institutionalization.

bullet

Senior citizens and persons with disabilities have positive social relationships and enhanced living conditions.

bullet

Informal caregivers increase their skills and support network.

bullet

Families understand and can access appropriate housing and care options for the elderly and disabled.
