February 11, 2012 – Request Form Submission Date
April, 2012 – United Way Community Impact Committee Grant Review Period
May 17, 2012 – United Way Board Review & Approval of Grants
May 31, 2012 – Agency Notification of Approved Grants
June 30, 2012 – First Payment
August 31, 2012 – Second Payment
October 31, 2012 – Third Payment
December 29, 2012 – Fourth Payment
February 28, 2013 – Fifth Payment
April 30, 2013 – Final Payment

Which Form Do I Use?

United Way St. Croix Valley will not conduct face-to-face reviews during this grant cycle. All previously funded nonprofit agencies should use the Interim Request Form link.

School-based programs should use the School Request Form. The amount of grant money available for each district is based on the amount of unrestricted donations made by staff and employees. District Superintendents will be notified of the amount available once all school campaigns are complete. Complete details are available at the School Funding Announcement.

Several questions on the form relate to “desired outcomes” for the program. Use the list of Outcomes ranked in order of United Way’s priorities for the category.

After completing the form, mail or deliver two unbound copies with attachments to:
