Which Form Do I Use?

• United Way St. Croix Valley will conduct face-to-face reviews of the major agencies in the “Basic and Emergency Needs” category this year. Those agencies should click the Request Form link.
• All other previously funded nonprofit agencies should use the Interim Request Form link.
• School-based programs should use the School Request Form. The amount of grant money available for each district is based on the amount of unrestricted donations made by staff and employees. Complete details are available at the School Funding Announcement.

Several questions on the form relate to “desired outcomes” for the program. Use the list of Outcomes ranked in order of United Way’s priorities for the category.

After completing the form, mail or deliver two unbound copies with attachments to:

Applications must be postmarked or delivered to the office by 4:30 p.m. on February 5, 2010.
